Community Care supports a range of primary care and community‑based services delivered through our network of medical centres across Aotearoa New Zealand. These services are designed to improve access to care, support wellbeing, and help people manage their health closer to home.

Many services are free or low‑cost, and availability may vary by medical centre. Your local medical centre can let you know what services are available to you.

Mental health and wellbeing support

We provide free, accessible mental health and wellbeing support through primary‑care‑based services. This support focuses on helping people manage everyday challenges and make positive changes that support their overall wellbeing.

Appointments are brief, practical, and focused on what matters most to you. Bookings are available through your local medical centre.

Support in this area may be provided by:

  • Health Improvement Practitioners (HIPs)
  • Health Coaches

They can help with:

  • Stress and anxiety
  • Low mood
  • Sleep challenges
  • Addictions
  • Building healthier habits
  • Setting wellbeing goals

Team‑based models of care

Our medical centres work using team‑based models of care, bringing together a range of health professionals to ensure people receive the right care at the right time.

By working together, care teams can improve access and provide more coordinated support for patients and whānau.

Your care team may include:

  • Extended Care Paramedics (ECPs)
  • Clinical Pharmacists
  • GPs, nurses, and other health professionals

This team‑based approach supports:

  • Assessment and management of urgent, complex, or acute health needs
  • Medication reviews and optimisation
  • Care delivered closer to home

We offer:

  • Patient Portal (The Doctors App) - to book appointments, request prescriptions, and view test results
