Mō mātou

Community Care is a Primary Health Organisation (PHO) supporting a national network of medical centres across Aotearoa New Zealand. Commencing operations on 1 July 2026, we support 54 medical centres with an enrolled population of 350,000+ patients.

We were established to strengthen primary care through shared infrastructure, consistent governance, and a direct funding relationship with Health New Zealand – enabling medical centres to focus on delivering high‑quality care for patients and whānau.

Our why

Community Care was established in response to sector change and the increasing pressures facing primary care. We exist to strengthen medical centres for the future by ensuring resources are directed where they have the greatest impact - at the frontline of care.

As a Primary Health Organisation, Community Care has the scale, infrastructure, and national reach to support high‑quality primary healthcare across the motu. Our approach focuses on directing funding to frontline care and reducing unnecessary administrative complexity, enabling practices and clinicians to focus on delivering care that meets the needs of patients and whānau.

Through our national network, experienced clinical workforce, and commitment to innovation, we support accessible, patient‑centred care while enabling strong and sustainable practice performance.

Our beginnings

Before Community Care was established, our network of practices operated across multiple PHOs, each with varying systems and approaches. Bringing practices together under a single national PHO enables more consistent service delivery, streamlined operations, efficiencies through scale, and clearer alignment with national health priorities.
